Hi I was wondering what happens if I want to add a belt to my acoustic, which does not have that tiny metal spike to hold the belt. Will they drill through the body? And will that affect the sound of my guitar?

you mean a strap and a strap button
if you dont have a strap button on both sides of the body you just tie a string to your headstock behind the nut and connect it to your strap
you can get a stap button installed into the heal of your neck joint that wont change any sound quality
