I think the key change in the last verse is up to C# not D. Try this - I think it's right

At the end of the verse the chord sequence is C / Am / | F / G / | C / / / | G / / / ||

At the key change this changes to C /Am / | F / G / | C / / / | Ab / / / ||

Then up to C# for the next verse

THERE IS A KEY CHANGE AS THE 3RD VERSE IS REPEATED. I CAN MATCH IT BY SLAPPING A CAPO ON ONE. ANY OTHER IDEAS I'M KIND OF A NOVICE.
