PS: Good job on the chords, but I think that C9 should be Bbdim7.

I believe this to be mostly correct. Use a root 5 barre chord form 
for the Em7 & Fm7 and a root 6 barre chord form for A13, Bb13, 
Bbdim7, and Bdim7.  Corrections are welcomed.
