Historically speaking, it hasn't been easy for tourists to go to Russia.  If you intend to travel overland between Kaliningrad and other parts of Russia and you're a visa national, you will have to have no less than a normal double entry visa so as to exit and re-enter Russia.  To enter Russia you are going to require a visa before travel.
