"F#/D 2 0 0 2 3 2 (real name?)"
You're close. It's one variation of D/F# (or a D with an F# as the root). Normally, though, for D/F#, you would mute the A string, showing it as 2x0232. However, since D contains A anyway (G string fret 2), it doesn't matter. Bottom line: Call it a D/F#.
